Terms of Service
Last updated: September 29, 2026
1. Agreement
These Terms of Service (“Terms”) govern your use of the MyRxProvider website and telehealth platform (the “Platform”), operated by [LEGAL ENTITY NAME], [MAILING ADDRESS] (“MyRxProvider,” “we,” “us”). You accept these Terms when you tick the Terms of Service box and submit an intake, or otherwise use the Platform. If you do not agree, do not use the Platform. Our Privacy Policy & Notice of Privacy Practices, Telehealth Informed Consent and GLP-1 Medication Informed Consent are part of these Terms.
2. MyRxProvider is not a medical provider
MyRxProvider provides a technology platform and administrative services. Medical care is provided by independent, U.S.-licensed clinicians who use their own professional judgment. MyRxProvider does not practice medicine, does not interfere with clinical decisions, and does not guarantee that any medication will be prescribed. Completing the intake does not by itself create a clinician–patient relationship.
3. Eligibility
- You must be at least 18 years old. The intake asks for your date of birth, and our server rejects intakes from anyone under 18.
- You must live in, and be physically located in, a U.S. state where the Platform operates at the time of each telehealth visit.
- MyRxProvider is a cash-pay program. Participating employers may refer their employees to it, but it is not insurance or a company-provided health plan. An employer access code is optional; you can apply without one. Every applicant completes the same full medical questionnaire, and a licensed clinician decides whether treatment is appropriate.
- You must give true, accurate and complete information. False health information can endanger your health and is grounds for ending your use of the Platform.
4. Not for emergencies
The Platform is not for medical emergencies. If you think you have an emergency, call 911 or go to the nearest emergency room.
5. Prices and billing
- No charge unless approved. The intake is free. You are not charged unless a licensed clinician approves treatment and you choose a plan. If treatment is not approved, you pay nothing.
- Prices. Current prices are shown on our Plans & Pricing page and again before you are charged. The monthly price depends on the treatment and the program length you choose (12-month, 6-month, 3-month or monthly auto-refill). The monthly price may also vary with the dose your clinician prescribes. Your exact price is shown and confirmed before you are charged, including before any price change when your dose changes. Prices include clinician visits, medication if prescribed, supplies and shipping. There are no hidden fees.
- How recurring billing works. Every plan is billed monthly. After approval, your payment method is charged the monthly price for your plan, then automatically once a month on the same date for the program length you chose, until the program ends or you cancel. Before your first charge we show you the price, how often you will be charged, the program length and how to cancel, and we ask for your agreement.
- End of a program. When your program length ends, billing stops unless you choose to continue. We will email you before your program ends with your continuation options and their prices, and we will not enroll you in a new program or charge you a different price without your express agreement.
- Price changes. Your monthly price does not change during your program. For monthly auto-refill plans, we will tell you about any price change at least 30 days before it applies, and you can cancel before then.
- Cash-pay only. MyRxProvider is a cash-pay service. We do not accept or bill insurance, Medicare, Medicaid or any other health plan.
6. Cancellation (click to cancel)
- Cancelling is as easy as signing up. You can cancel any plan at any time, online, from your patient account, in no more steps than it took to sign up. You will never be required to call us, email us, chat with anyone or sit through a sales offer to cancel.
- You may also cancel by emailing support@myrxprovider.com if you prefer, but you never have to.
- We confirm every cancellation by email.
- Cancellation stops all future charges and shipments and takes effect at the end of your current monthly billing period. You keep any medication already dispensed for that period. There is no cancellation fee.
- If you are on active treatment, your care team can help you stop safely. Talk to your clinician before stopping a medication.
7. Refunds
- Compounded medication cannot be returned once dispensed. For safety reasons, and under pharmacy law, a pharmacy cannot take back or re-dispense a compounded prescription after it has been prepared for you. Charges for medication that has been dispensed are therefore not refundable, except as described below.
- Damaged, incorrect or lost shipments: tell us within 7 days of delivery (or of the expected delivery date for a lost package) and we will replace the medication or refund that charge.
- Treatment not approved or stopped by your clinician before dispensing: if you were charged for a period in which no medication was dispensed because a clinician did not approve or stopped treatment, we refund that charge.
- Refunds go back to your original payment method. Nothing in these Terms limits any refund right you have under law.
8. Prescriptions and pharmacy
Prescriptions, if issued, are filled by independent, licensed U.S. pharmacies and shipped to you. You may ask for a prescription to be sent to a pharmacy of your choice instead; plan prices apply only to fulfilment through the independent, licensed U.S. pharmacies MyRxProvider routes prescriptions to. Medication may be used only by the person it was prescribed for. Do not resell or share medication. Compounded medications are not FDA-approved; see the GLP-1 Medication Informed Consent.
9. Communications
You agree that we and your care team may contact you by email, phone and text message about your intake, care, prescriptions, shipments and billing. Message and data rates may apply; reply STOP to opt out of texts. Marketing emails always include an unsubscribe link and our mailing address, and we honor unsubscribes within 10 business days. See the Privacy Policy.
10. Patient account security
If you have a patient account, keep your login details confidential. You are responsible for activity under your account. Tell us immediately at support@myrxprovider.com if you suspect unauthorized use.
11. Acceptable use
You agree not to:
- try to obtain medication through false statements, or use another person’s identity;
- copy, reproduce, republish, frame, mirror or create derivative works from the Platform’s design, layout, text, graphics, photographs, product images, logos or code;
- scrape, crawl, harvest or extract content or data from the Platform by automated means (including bots, spiders and AI training tools), except ordinary search-engine indexing that follows our robots.txt;
- reverse-engineer, probe, overload or interfere with the Platform, its security checks or its forms; or
- use the Platform for any unlawful purpose.
12. Intellectual property
The Platform, including its design, look and feel, text, graphics, photographs, product images, logos, the MyRxProvider name and the underlying code, is owned by MyRxProvider or its licensors and is protected by copyright, trademark and other laws. We give you a limited, revocable, non-transferable license to view and use the Platform for your own personal, non-commercial purposes. Any other use, including copying the site’s design, content or images for another website or business, requires our prior written permission.
13. No guarantee of results
Weight-loss results vary from person to person and are not guaranteed. Any results described on the Platform are based on published clinical studies or individual experiences and are not a promise of what you will achieve.
14. Disclaimers
THE PLATFORM IS PROVIDED “AS IS” AND “AS AVAILABLE.” TO THE MAXIMUM EXTENT PERMITTED BY LAW, MYRXPROVIDER DISCLAIMS ALL WARRANTIES, EXPRESS OR IMPLIED. NOTHING IN THIS SECTION LIMITS A CLINICIAN’S PROFESSIONAL OBLIGATIONS TO YOU OR ANY LIABILITY THAT CANNOT BE DISCLAIMED UNDER APPLICABLE LAW.
15. Limitation of liability
To the maximum extent permitted by law, MyRxProvider’s total liability arising out of or relating to the Platform will not exceed the amounts paid by you for the service in the twelve months before the claim. This limitation does not apply to claims of medical malpractice against treating clinicians, gross negligence, willful misconduct, or any liability that cannot be limited under applicable law.
16. Governing law and disputes
These Terms are governed by the laws of [GOVERNING-LAW STATE], without regard to its conflict-of-law rules, except where the law of your state of residence gives you protections that cannot be waived. Before starting any formal claim, please contact us at support@myrxprovider.com so we can try to resolve it informally. Disputes that cannot be resolved informally will be resolved in the state or federal courts of competent jurisdiction, unless you and MyRxProvider agree in writing to arbitration. Nothing here prevents you from bringing a claim in small-claims court or filing a complaint with a government agency.
17. Changes
We may update these Terms from time to time. The current version is always posted here with its effective date. We will tell you about material changes by email or on the Platform before they take effect. Changes to prices or billing never apply to a charge you have not agreed to.
